Applicants should submit to the Registrar (unless otherwise noted) the following materials:
- A completed application for admission. Electronic submission preferred.
- A $30.00 non-refundable application fee (pay online when you are in the system or send check or money order payable in U.S. dollars drawn on a U.S. bank in the U.S. to: Office of the Registrar, UTHSC-H, P.O. Box 20036 Houston, Texas 77225). Note: The application fee is not due by the deadline for your program.
- One official copy of your transcript from each institution attended. If you have attended any school within The University of Texas Health Science Center at Houston it is not necessary to request a transcript. Electronic submission preferred. Transcript(s) should show:
- A Baccalaureate degree in nursing earned from a nationally accredited program.
- A Master’s degree or Post-masters certificate as a nurse practitioner from a nationally accredited program with a GPA of 3.0 or above.
- A completed graduate level statistics course within the last 5 years.
- A completed graduate level nursing research course within the last 5 years.
- Three (3) letters of reference attesting to the applicant’s academic ability and potential (including one reference from a professional colleague). Download copies of the Applicant Letter of Reference Form and provide them to individuals who you have requested to provide a letter of reference.
- One official copy of TOEFL score if international applicant. TOEFL scores can be no more than two (2) years old and a minimum score of 550 is required on the paper exam, a minimum score of 213 on the computer-based exam, or a minimum score of 86 on the internet-based exam.
- A course-by-course evaluation of international credentials by Educational Credential Evaluators (ECE) for individuals with degrees from outside the U.S.
- Eligibility for or current licensure to practice as a registered nurse in Texas.
- Eligibility or approval to practice as a nurse practitioner by the Texas Board of Nursing.
- Current certification by a nationally recognized credentialing organization as a nurse practitioner.
- Documentation on application of a minimum of one (1) year work experience as a nurse practitioner.
- A 500-word typed statement of area of practice interest, career goals, and reasons for desiring doctoral study. Email to soninfo@uth.tmc.edu or mail to: Student Affairs Office, Attn: Admissions, The University of Texas School of Nursing at Houston, 6901 Bertner Avenue, Houston, Texas 77030.
- A curriculum vita that documents practice expertise, scholarly activities, and publications.
Additional Requirements:
- Qualified applicants will be interviewed by faculty members to provide an opportunity to assess whether the program interfaces with applicant’s educational needs and career goals.
Additional Requirements (if admitted into program):
- Acceptance is contingent upon successful completion of a course in epidemiology, a graduate level statistics course and a graduate level nursing research course, immunization screening, immigration status clearance, Health Care Provider Basic Life Support (CPR) certification, and clearance of the mandatory criminal background check. All new students must successfully clear a criminal background check prior to enrollment.
